Watch for your intake forms email — and fill them out
You will receive a separate email from CarePlatron shortly with a link to your patient intake forms. These cover your personal info, medical history, current medications, and your goals for this work. Completing these forms is the most important next step — please fill them out as soon as you receive them.

Book your medical consultation
Once your intake forms are submitted and reviewed, you'll receive a separate email with a link to book your medical consultation — a 30-minute video call with our MAPS-trained nurse practitioner. This is required medical clearance before any ceremony work, and the booking link is sent only after your forms are in.

Your deposit is fully refundable
If our clinician determines you are not a medical fit, your {{deposit_amount}} is refunded in full within 3–5 business days. No questions, no friction.
